- Physical examinations

- Respond to your requests and queries

What type of information we collect
- We collect your personal information like name, address, telephone number, mobile phone number, email address, company name, business registration, Tax Identification Number (TIN), Philhealth Number and other details that are necessary for the usual business-to-business transactions.

Where we get these information
The personal information we have are those given by yourself when you registered as a client/patient or when you take part in our research or surveys. We may also collect information from publicly available sources.
We will never share, rent or sell your personal information outside of PUDCI.
How we protect your information
We strictly enforce our privacy policy within PUDCI and we have implemented technological, organizational and physical security measures that are designed to protect your information from unauthorized access, use, alteration or disclosure.
We restrict access to your information only to qualified and authorized personnel who hold your information with strict confidentiality.
We keep and protect your company and personal information using a server secured with a firewall, antivirus software, and database encryption.
We keep printed documents with your information in designated boxes, filing cabinets and storage rooms accessible only to authorized individuals.
We undertake regular audit and rigorous testing of our data security protocols to ensure your data is always protected.
We will retain your company and personal information throughout the duration of your relationship with PUDCI and for a period of five (5) years after you ceased to be a client/patient.
